My 96 eldorado has a very loud ticking and rattling sound coming from the driver side rear wheel only when backing up. I looked at the back rotors and they look worn, especially the driver side rear rotor, where the noise is coming from.

I plan to replace the rear brake pads and rotors, but could this be a different problem? I can also hear a squeaking noise when driving like when the brake pads are low, which sound's like it's coming from the rear.

Noises backing up usually come from anti rattle shims missing or glazed rotors. Its not unsafe just annoying. When you need pads, make sure the anti rattle shims or clips are installed and most times you simply resurface the rear rotors unless they measure too thin to do so.
