Sadly, it's not even close. A Nazi ship was torpedoed in the closing days of WW2.. as many as 9,300 people may have been killed. It was carrying primarially civilians.

Personally I wouldn't consider a ship being torpedoed in wartime as an accident. That's war. Even if it's a civilian ship, they were known to transport supplies.

CIWS, I was gonna say the same thing...a torpedoed ship is not an accident.
