I bleed with the engine off. It will bleed all fluid except that in the abs pump. No need to flush the pump IMO as there is so little fluid and it will get recirculated when the abs kicks in. If you have cracked open the system (replaced any lines) you should bleed the abs pump. I've done that two ways. The best is hook up to a Tech II and open the pump circuit and bleed. The other way is to do a full bleed, take the car out on a dirt or otherwise slippery road and hit the brakes so the abs goes on. Then bleed again.
Not much to add, I'll just say I've used a power bleeder with car off. Works fine...
