actually - not an unusual problem -

MOST of the time it's worn out brushes within the motor -
which means simply replace it -

SOMETIMES a good smack with the butt of a screwdriver may start it working again -
for a very short time that is -

SOMETIMES it is a ground problem -
the ground connection is hidden behind the passenger kick panel -

but typically you get a sporadic fan -
works until you hit a bump - or make a turn -
or stops working after hitting a bump - etc -

RARELY - it could be a problem with the plug itself -
inspect the connectors inside the plug for evidence of arching - or melted rubber -

if it turns out to be the blower itself -
you can save some $$$$$$$$$$ -

    I recommend the TYC 700110 -
    $40 - with free shipping - on ebay -

or if you prefer amazon -
    $45 - with free shipping -

Same problem with my 2004 took access panel off on pass side and bumped it started turning but not sounding good. I took mine off and took it apart to find the motor piled up with black brush dust and plenty of brush left. cleaned out the dust with air nozzle outside put it back in works ok for now. basscat has good info on replacement advance and autozone want $139. Good luck!

Did the exact same thing on my daughters '04 SLS a few months ago with the same results. It's been flawless ever since.
