You just do, and plan the best you can for what might happen. Even with a run flat, situations on the road could occur that damages the wheel/tire enough that it's not drivable. Then the same type situation would probably require calling Roadside assistance in some form.
Besides a better ride and wider selection the tires are usually several hundred dollars cheaper, and that can pay for years of AAA type assistance.
